Authorization forms and medical history forms are REQUIRED to be filled out prior to your appointment.

If these forms are not done out prior to drop off, you will be required to fill out a print out of each form.

By completing these forms, you also acknowledge you have read, and understand any vaccination or surgical protocols and aftercare instructions.

• Medical History Forms (Surgical AND Vaccine Appointments):
  • Medical History Questionnaire (one for EACH animal being seen)

**Please Note: Any postoperative or post-vaccination complications due to failure to follow our aftercare instructions may be referred to an emergency clinic or treated at our facility at the owner’s expense. This will be determined on a case-by-case basis by our veterinarian.
